Take a look at Midiboek at http://huizen.daxis.nl/~Ppaardekam/ a
program to convert MIDI input to paper templates for punching music
rolls/books. It has also been used to drive a punch from the .prt file.

Also look at Noteur, an editing program for mechanical music, at the
same site. Both are freeware.
