Dear TJMC and parents:
I just wanted to highlight perscription medication information for TJMC band
camp at Orkney Springs...please note that the requirements have changed
since last year!
If a student is bringing perscription medication to band camp, a physician's
signature is required on:
Form 1 - Medical Authorization Release/Indemnification Agreement
AND
Form 2 - Medical Information
Both forms are included in the TJMC information packet that was sent
out in late July
A parent MUST accompany their student during check in at TJ (Sunday, Aug 22,
9:00 a.m.) and must turn in all forms at that time. No student will be
allowed to board the bus for camp unless all forms are properly completed
(including a physician's signature for perscription med's). We will have
extra forms and volunteers on hand to help with completing forms, but you
could have great difficulty getting your physician's signature on that
Sunday morning, so please take care of this ahead of time.
This requirement is in accordance with FCPS Directive 2102.9 which went into
