It might not be it, but have you connected the diode near the
optocoupler in the right direction?

The mark on the 6N138 is there, just hard to see in that picture. I turned it around just to doublecheck, but it did not resolve it so I put it back. Thanks...

It looks like your chrystall might be too far away from your PIC. I had the same problem come to think of it, and i remember it was murder getting it all back...
