How frustrating. Every part of the midi circuit checks out. I have continuity
in all the right
places. I replaced the 6N138 and I tried a different midi connector as well.
The thing that
really makes me think midi is the issue is that when I set the unit to midi
learn mode it
perpetually waits. I can send one note blasts to the gameboy with the
plus/minus buttons or
by changing to midi full or keyboard modes, so I know the rest of the circuit is
working. I
keep thinking it must be something stupid. Pix up soon.

It looks like your chrystall might be too far away from your PIC. I had the same problem come to think of it, and i remember it was murder getting it all back...
