Check again,
turn the diode around again, try to swap the 2 midi-lines and finally,
replace the 6n138. Also check if the line from the 6n138 is connected on
the right pin in the PIC. I hope you are also using a 20Mhz quart. (
not a slower one i mean).
Send us some good pictures, maybe we can reverse engineering it. :)
