The diode near the opto-coupler is turned correctly, so that's not the
problem. Have you tried putting the 6N138 the other way? Weird enough,
it seems that your 6N138 doesn't have the jack to show which way it
should be connected.
Another thing I'm noticing is how the resistors you are using looks.
These are high wattage diodes, which might interfer with the signals.
While high wattage diodes are good for circuits with high currents,
they should typically never be used in a signal path. The reason for
this is because the wire is wired around the center, effectively
making it a coil, in other words a sort of lo-pass filter. This might
filter the MIDI signal alla away.
You should use 1/4 W or 1/2 W resistors. The only resistors you should
need to replace is R1,R2 and R3 (220, 5k6 and 1k2)

The mark on the 6N138 is there, just hard to see in that picture. I turned it around just to doublecheck, but it did not resolve it so I put it back. Thanks...

It looks like your chrystall might be too far away from your PIC. I had the same problem come to think of it, and i remember it was murder getting it all back...
