Dear organ enthusiasts,

In the process of learning Petr Eben's gigantic "The Labyrinth of the World
and the Paradise of the Heart", I am looking for the tune name and text of
the attached chorale which he uses in one of the movements.

I would dearly appreciate it if any fellow organaut can come up with the
answer to this puzzle.

Here it is in pdf. I suspect that it comes from a Bohemian Brethren and/or
Moravian source.
Does anyone have such hymnals at hand or have an idea where I could see a
thematic catalogue of same?
