Here is the way I wired my 4 way for my tele with two HBrs. Hope you can use it?
I put a PP pot on it to coil shunt the neck and I LOVE IT!

Yes, I reattached the ground wire. I get no hum at all from either of the hot
rail pickups (neck and middle) it is only the bridge. Which makes me think I
wired something wrong. That's why I was hoping for a diagram.
