did you check the voltage on the chip incommer it should be between 7-17 vdc it does look like its the dac the second sound channel buffering opamp is not...
check which output is functioning on the dac pin 9 channel1 for 0-5v being analog voltage then check pin 10 ch2 for the 0-5v analog voltage which one is not...
